Ngā Poutama: Consumer, family and whānau experience survey

Ngā Poutama: Consumer, family and whānau is a survey about your experience with mental health and addiction services provided by district health boards.  This survey is being run by the Health Quality & Safety Commission with help from Mobius Research and Strategy.

By taking part in this anonymous 15-minute survey, you can help us learn what is working well, and where improvements can be made.

We will share the survey findings with consumers, families, whānau groups and providers.  The information will not contain any details that could identify you.  Since the survey is anonymous, we are unable to change your answers for you once it is submitted.
